Anesthetic Management for Imparied Liver Function Patient Case Report of Patient with Budd-Chiari Syndrome


Abstract
Budd-chiari syndrome is a rare syndrome that results from thrombosis of the major hepatic veins. In addition, this syndrome can be caused by retrohepatic venocaval obstruction and by intrahepatio outflow obstruction, usually tumor.
And portal hypertension eventually develops. Prognosis, regardless of etiology is generally poor, fewer than 30% of the patients surviving longer than 12 months. And mortality in surgical patients with Budd-Chiari syndrome is over 30%. The degree
of
hepatocellular function encountered will be variable depending on the time between onset of pathologic and diagnosis and these patients will have far more hepatic reserve than will the cirrhotic patient with portal hypertension. The correction of
coagulopthy, electrolyte imbalance, and hypoalbuminemia should be done pre-operatively. And special anesthetic considerations are needed to avoid post-operative hepatic dysfunction and complications.
